Form Form Details
Administrative Withdrawal Students who have experienced severe, extenuating circumstances and were unable to successfully withdraw themselves from specific courses before the withdraw deadline may request an administrative withdrawal. If approved, an 鈥淎W鈥 designation will be recorded on the student鈥檚 official academic record and no academic credit will be awarded. Students who have successfully withdrawn or who are able to withdraw before the published deadline, should not request an 鈥淎W鈥.聽For more information,聽.

A student audits a course for informational purposes only. If space is available, students who meet 海角社区 admission requirements and are formally admitted to the University may audit a class with permission of the instructor. The cost for auditing a course that takes place on the main campus is based on applicable tuition and fees.

Persons aged 60 and older may audit courses at no cost through the Meritus Scholars Program. For more information on this program, visit the Center for Individualized Learning website.

Course Overload Petition (Non-CLAS Courses ONLY) **This form will require the use of Adobe Reader to fill out the PDF. See above on how to access Adobe Reader.**

Declaration of Major/Minor **This form will require the use of Adobe Reader to fill out the PDF. See above on how to access Adobe Reader.** Change your major, minor, catalog year, and/or certificate program. Students must complete this form with their academic advisor.

NOTE: Placing a rush order on a replacement diploma order only means that the delivery service will FedEx Overnight instead of the regular mail through USPS. This does not expedite the processing and printing of the order.

Apply for certain semesters to be removed from GPA and academic credit. A Fresh Start will not change Financial Aid satisfactory academic progress evaluation.

Students are REQUIRED to work with their academic advisor to complete this form.

Once you have applied for graduation, if you receive notification that there is an adjustment that needs to be made to your application, you will need to complete this form.

Deadline: Dates vary by term, please reference the for specific dates and deadlines.

Apply for more Institutional COF hours after you’ve reached your 145 COF hour limit.

Request enrollment into a course after the census date for the course has passed.

DO NOT SUBMIT THIS FORM IF:

  • You are waiting on approval for a Participation Reinstatement decision
  • You are requesting to be re-enrolled into a course you accidentally withdrew from.

This form will only be open after the end of the current semester and is only to be used by those who missed applying for graduation this term.

All students must formally Apply For Graduation online through their Student Hub for each degree and/or certificate.

Students will be notified if their late application has been accepted via email.

Request a letter confirming you never attended 海角社区.

Letters confirming non-attendance are for individuals who NEVER attended 海角社区. For students who attended previously, but are not currently enrolled who need proof of their records, follow the steps to order an official transcript through Parchment.

The Office of the Registrar is no longer collecting Walk Only requests for Commencement.

Any student who wants to participate in a Commencement ceremony must Apply for Graduation for that semester through your聽.听**Summer graduates-you will automatically be invited to participate in the Fall Commencement ceremony and you do not need to contact us to participate in the Fall ceremony.听听If your degree has already been awarded and you聽now聽 want to participate in commencement ceremony please contact the Degree Evaluation Team at聽[email protected]

海角社区 has authorized the National Student Clearinghouse to provide official Enrollment Certificates.听 The Enrollment Certificate provided by the National Student Clearinghouse is official and does not require an additional signature or seal from 海角社区. The verification can be used for proof of enrollment for employers, military, insurance companies, housing, scholarships, and credit card companies.听 The official document includes:

  • Enrollment Status (full-time, part-time, etc.)
  • Dates of Attendance
  • Expected Date of Graduation
